Seah Hong,

Look in the ORACLE_HOME for the CRS software in the crs/log directory.  
The log file there may have some clues.  Also important is the rdbms
ORACLE_HOME/racg/log directory and possibly the
ORACLE_BASE/admin/$ORACLE_SID/hdump files.

Something else that may be helpful.  Create a $ORACLE_HOME/log directory
in the CRS ORACLE_HOME.

Usually the cause is a system problem where racgimon takes to long to
connect to the database to check its health so it shuts it down.  There
is a few bugs on this thay may be at play.  One of them may be issue
3764216/.
